Text first, no heading, no auto TOC works OK.

__TOC__ or {{rtoc}} as first thing with no page title or headings works OK for preview.

__TOC__ or {{rtoc}} followed by text followed by L1 Heading has a problem.

L1 Heading as first thing, with no TOC or rtoc has a problem. No TOC is automatically displayed either.
